I'd say the most disturbing is the remake of The Hills Have Eyes. A mother getting shot and eventually eaten. Another mother getting shot in the head. A girl getting raped by a mutant. Father getting burned alive...
Yeah, that's a fucked up movie.

That was a pretty fucked up movie when I first saw it. I was in disbelief that someone's head had actually been cut off (not to mention the other stuff).
It's rather scary that nowadays people wouldn't even flinch unless they SAW all the blood, gore, and knife-dildo rape.

August Underground's Mordum.
I could only watch 20 minutes of it, if you've seen Cannibal Holocaust- this is 50x worse.
EDIT: August Underground is the one I half saw, Mordum is the sequel- I haven't and will not watch it.
I thought mordum was just too over the top to the point where it became corny and pointless. It was more like a faces of death video than an actual movie.

for some reason, Pet Sematary always creeped me out...I think it was the thought of being chased down a hallway by the sister with spinal meningitis that did it for me. Having watched the movie more recently, it's not as disturbing as it was the first time...
I'd also say that Event Horizon was pretty disturbing. They implied that the ship went to hell and came back, but we'll never really know for sure...and the thought of it is, for lack of a better word, disturbing.

The Girl Next Door
It is a true story about a near preteen girl that gets tortured, stripped and raped by her own family and other neighborhood kids. The poor girl dies after her aunt burns her pussy out with a propane torch.
It was really hard to watch and I don't think I ever will again.

can't say i've seen mordum, but i have seen cannibal holocaust. that shit terrified me when i was younger, but i saw it again recently and it wasn't nearly as bad. it's still very disturbing though, the music/sound effects are extremely effective, as are the camera angles.

First time I saw it I was like 12 and was fuck scared out of my face. But I loved it.
The Devils Rejects is more fucked up though because it's more realistic. Apparently when they were filming the rape scene a lot of the backstage cast was crying.

Cannibal Holocaust is something everybody should watch for the gross/gore factor, but it doesn't look as realistic these days as it did back then.

Disturbing for most people,hilarious to me.
And i'm not even trying to say "hilarious" to sound like an e-badass. I actually did laugh at parts of this movie.

I watched that for the first time since I stopped smoking weed a couple months ago and I thought the movie was fucking terrible. It used just about every horror movie cliche in the fucking book, I don't even watch horror movies and I could see most of the shit coming. The acting wasn't very good, the lead female kinda reminded me of Dakota Fanning, almost constantly screaming and yelling.
The fat old lady zombie at the start was fucking hilarious and that part where they're heading up to the penthouse and the fire marshall guy helps them escape, then they turn the camera around and the guy is now a zombie and does this fucking hilarious look... man, it's fucking priceless.
The only part I thought had a good horror atmosphere was the part right at the end in the penthouse when the lights go out and it switches to night vision and the zombie chick with the hammer enters the room. Before that when they're looking at the articles on the wall and listening to a recording and the chick is yelling about some shit annoyed me, once that stopped it got good. Then went to the predictable ending where the reporter is pulled out from under the thing she's hiding under.
I will say that the make up and shit in the movie was good, the bite marks looked great. Otherwise I consider it another movie that is considered "new and different" for the sole reason that it's not in english. But yeah, I'd give the movie a 2 out of 10.

I've seen every single David Lynch, Takashi Miike and Alejandro Jodorowsky flick, as well as Salo, Zoo, Men Behind the Sun, Cannibal Holocaust, Man Bites Dog, August Underground, I Spit on your Grave, Tale of Two Sisters, Slaughtered Vomit Dolls and countless other shock cinema/snuff/various horror flicks and I can say that it should probably go to Irreversible. It wasn't that it was hard to watch, or that it really disturbed me (what does anymore. We are all so very jaded.) But it's realism really hit hard. I couldn't even figure out how I should feel about the 5-10 minute rape scene. Every single time I get to that part I am shocked at how erotic it was to me. And that's the true, raw disturbing part of any movie that has the courage to really dig into the filth- the audience wanting to watch it.
In the end, most of the films suck. I love Lynch and Jodorowsky, but Miike is a hack. He's up there with Eli Roth.

cannibal holocaust was disturbing the first time i watched it, same with salo. i spit on your grave wasn't really disturbing, it's just some whack feminist revenge fantasy caught on camera. irreversible was pretty full on i agree. i remember hellraiser 2 (extended version) being very whacked out and disturbing when i first saw it, especially when the junky gets skinned alive. such a weird movie, it was like watching a really bizarre nightmare. hellraiser one was also quite disturbing

As for Lynch films, my favorite will always be Blue Velvet, and I'm a Holy Mountain guy myself. But very cool, its rare I ever run into anyone that mentions those two names in a single sentence.

I was pretty young the first time i saw it as well. It was one of those movies where i had to ask myself, "WTF did i just witness?"
Devils Rejects had less of the "cliche American slasher film" feel to it. I think that's what made it more realistic.
And it wouldn't surprise me if the cast was crying. That scene owns.

I have only been able to watch about 15 minutes of it, but my friends who have survived (about 4 of them i thinks.) , said it was the most intense, horrible movie ever.

I vote for Irreversible. There's just something about it. The 'end' (it's told backwards, and it actually works really well in the format) is so poignant and affecting given what's come before. Real fucked-up film. Difficult viewing.
I've only seen Devil's Experiment out of the Guinea Pig series. Difficult watching at times, but there's nothing in there that's any worse than stuff on liveleak or TheYNC or sites like that. Gore and violence isn't particularly shocking to me any more; in my eyes, for something to be disturbing, there has to be emotional content.

Thats what was interesting about Funny Games, theres not really any gore and little violence on screen but the movie keeps you uncomfortable throughout.

Definitely Eraserhead, Happiness, and Irreversible.
Also, Un Chien Andalou is very strange. A movie made by Salvador Dali (remember the melting clocks?) and Luis Bunuel that was inspired by dreams they each had. Pretty shocking for 1929, seeing as it shows scenes such as a woman's eye being sliced open with a straight razor. The film jumps from "3 AM this morning" to "8 years later" to "16 years ago" and etc... What's more disturbing is the aftermath of the movie. The two lead actors killed themselves afterwards. One overdosed on pills, the other set herself on fire in a public market.
