When you go to the Home Depot, there is a tag on every shelf, under every product.
This is a large hang tag, they all essentially look like this, but the ones on the shelf are squished.

The SKU number, is the 6 digit number, separated by a hyphen, above the UPC. In this case 174-278.
In terms of the approved by, take a walk around your store. Near one of the entrances, or the Service Desk or even the Pro Desk, there will be a metal display that should have a picture of the store manager, and his/her name.
Or, you could go to the service desk, and ask who the department head of whatever department your item is from, is.
When you fill out the tag, write the initials, and make them look nice.
I.e. "Who is the Department Head of Hardware" for a Dremel.
