Why am I this way? Why can't I be something worthy of love and acceptance?
I found If I pretended to be someone else I could be loved.
But after awhile I realised the person they loved wasn't me.
I am no one therefore I am loved by no one.
So I decided to end myself.

You have to learn to accept/love yourself to a certain degree before you can expect others to reciprocate.
Pretending or attempting to be someone that is unnatural to yourself will only lead you astray. You can forget who you really are and may one day find that you have been chasing a image nonexistent outside the abstractions of your mind, only to become further aware of your emptiness. It is possible to alter oneself and adjust certain perspectives concerning, although you have to start from somewhere. Is it not more practical to move forward with what you already have as it will always be behind you?
Accept the fact that you will never be another human being, you will continuously develop and can change as an individual to an extent. It sounds as though your reliance on the outward perception of others has gained weight over the introspective focus of yourself.
A couple things to remember:
1.) Sometimes, if it feels as though a relationship is not working, it is because it is not meant to be. The pursuit and continuation of such has a fleeting chance of resulting in symbiosis.
2.) A lot of people are just plain assholes.
You aren't going to get a plan from me as that is something that you have to make for yourself. Somewhere to start, there has to be at least a few things that you like about yourself. Why do you think that it is that you are partial to these characteristics? Expand on them. What are activities that elevate your mood? Do them on a daily basis, invest yourself further, etc.
Stop trying to be accepted. Stop trying to be happy. Stop pretending. Rather focus on the development of yourself, and defining who you are.
Others will be forced to accept you, as that is the way that you are.
